Library Instruction Request Form

To arrange instruction for your class(es), please complete this form at least one week prior to the date requested (for High School instruction classes, click here). Remember that it is best to:

Provide all requested information to ensure a high quality session.
Explain your assignment (if applicable) under "General Assignment Description" so librarians can best prepare for your class.
Review Guidelines for Effective Library Instruction and Sample Instruction Sessions for incorporating instruction into your classroom assignments.
Schedule sessions with you in attendance. Library instruction is most effective when team-taught by the librarian and the class' instructor.
The Library Instruction Lab has only 12 computers. This can accommodate two students to a computer, or 24 students. Please consult with the Coordinator if your class is larger so alternate arrangements can be made.
